Situation #1
A nurse receives her shift in the orthopedic unit and in the report, she is notified that one of her patients refused the 9:00 am and 1:00 pm doses of pain medication. The patient has a diagnosis of fracture of the femur with external fixation. Early in the shift, the patient reports a lot of pain and asks for his ordered medication and is given 8 mg of Morphine intravenously at 4:00 pm.
The nursing professional needs to leave the unit and notifies his partner. During his absence, the patient communicates again complaining of pain and the nursing professional of the available unit decides to administer the ordered medication. According to the patient’s medical record, he is given Morphine 8mg intravenously at 4:25pm. When you return to the unit, you realize that a second dose of medication has been administered to the patient, in a short period of time. In the research carried out by the nursing leader, it was found that:
· The nursing professional in charge withdrew and administered the medication, but did not record it in the patient’s medical record.
· The medication was withdrawn (or removed from the narcotics box) but not documented in the narcotics log.
In this situation, it is important to remember the pain management medication administration process and the patient discharge process.

Situation #2
You are the nurse in charge of a patient who has presented with multiple symptoms. After several studies, the patient has been diagnosed with hepatitis C. After the doctor discusses the laboratory results with the patient, you proceed to educate the patient about the disease and the treatment to follow. In the process, the patient asks you to keep the diagnosis confidential with their relatives. Later, the patient’s wife asks to speak with you and the doctor because she wants to know the results of the labs and tests that were done. The wife is distressed and concerned about her husband’s health.
1. Identify the duties and responsibilities of the nursing professional in this situation in patient care.
2. Identify how you, in your role as an advocate for patients’ rights, would handle the situation and how you would maintain a boundary between personal and professional values, when exercising your role as a nursing professional.
