Pablo is a support worker who visits Sam for two hours, three afternoon per week to support him with his personal care and provide respite for Sam’s mother Karen.
Sam is 19 years of age and has an intellectual disability. Sam is fairly independent, he does however requires some support with his activities of daily living such as personal care. Sam is a happy young man, loves music and routine and finds change difficult to deal with. Sam has been known to display ‘behaviours of concern’ such as hitting out when he feels unsafe.
Sam lives with his mother Karen, and dog Axle. Karen uses these afternoons when Pablo is with Sam to do her own thing. Sometimes Karen catches up with a friend, goes to a movie, visits the hairdresser or does a workout at the gym.
Pablo has been providing support for about 12 months and has built up a good rapport with both Sam and Karen. Pablo knows what works well and what doesn’t and the routine that Sam is comfortable with.
Today Pablo enters the house and greets Sam and Karen. Sam is in another room, however Pablo can hear Sam pacing and yelling. Karen looks sad and exhausted. She tells Pablo she won’t be going out today. Pablo is concerned and asks Karen “Is anything wrong?” Karen tells Pablo about an incident that occurred this morning; Sam’s usual morning personal care worker was away, and a new personal care worker, Frank, had arrived to assist with Sam’s personal hygiene needs, and it didn’t go well.
Karen usually likes to help shower Sam as he sometimes displays challenging behaviours and has been known to ‘push and shove’ if his shower ritual isn’t performed in the way he likes. If Karen does things just the right way, Sam relaxes through the process and shower time is non eventful.
Today however when the new care worker Frank arrived, he came in and while well-meaning said to Karen “I’ve got it under all under control, I can shower a person with my eyes closed I’ve done so many, go and relax and leave it to the pro. Karen tried to protest but Frank insisted she take a break.
Getting Sam undressed and into the shower started out fine so Karen decided to leave them to it and go into the back garden to have a coffee. Before long Karen could hear Sam calling out to her, yelling that the wrong shampoo was being used. By the time Karen ran back into the house and into the bathroom Sam had become increasingly agitated, Frank appeared dishevelled and uncertain as to what to do. In a matter of seconds Sam had lashed out and hit both Frank and himself.
Sam has remained in an agitated state ever since, and Karen has not been able to calm him. Karen is stressed and exhausted and does not want to go out and leave Sam with Pablo in case he does something similar again. Pablo tries to reassure Karen, telling her “I am sorry you both had a tough morning Karen. What can we do to help settle Sam and help you relax and have some well-deserved time to yourself?”
Karen is relieved that Pablo understands the situation and wants to help. She says to him: “I am so glad you came today and I could talk about this. Sam loves spending time with you, you understand him. I think Sam would probably just like to put his ear phones on and listen to some music. I might just have a lie down, I don’t feel up to going out but some time to myself would be just great’.
