            My PICOT Question: In geriatric patients residing in long-term facilities who are currently taking multiple medications due to chronic conditions (P), how does the use of clinical medication reviews (each provider visit, fall, admission, re-admission (I) compared to regular conventional care (C) affect the outcome of polypharmacy prescription (O) within eight-twelve weeks period (T)? Clinical and medical reviews can reduce polypharmacy amongst the elderly population because clinical medication reviews (CMS) effectively reduce potentially inappropriate and unnecessary medications, patient-centered approach deters unnecessary medication.
