Mr. J.A has signs and symptoms synonymous with Idiopathic Cirrhosis of unknown etiology. Non-alcoholic associated fatty liver disease (NAFLD) also known as Metabolic (dysfunction) associated fatty liver disease (MAFLD) – which include Nonalcoholic fatty liver (NAFL) and Nonalcoholic steatohepatitis (NASH) as well as Autoimmune hepatitis, Genetic liver disease, Hepatitis B and C and Wilson’s disease. The Hispanic population’s cirrhosis and liver disease cases are mostly due to NAFLD (Hua et.al, 2021). The clinical presentation of weakness, jaundice eye, enlarged abdomen and weight loss strongly point NAFLD as the diagnosis. NAFLD is the leading type of chronic liver disease. It affects about 25% of the world population, of which 80% of the affected people are obese while 20% have normal weight. (Mayo clinic 2021)
The Liver’s basic functions involve breaking down both endogenous (bilirubin) and exogenous (drugs, alcohol) materials and removing them out of the circulation. The mechanism of this liver disease starts with the accumulation of fat in the liver leading to its inflammation and enlargement. Also, there is replacement of normal, healthy liver cells with degenerative nodules, fibrotic and Scar tissue. Similarly, vascular alteration causes diversion or shunting of blood flow from the portal vasculature to the central vein, resulting in portal hypertension and varices. Swelling (ascites) in the perineum leads to increased abdominal girth due to liver and spleen enlargement mostly in the right upper quadrant.(Karthikeya, 2022). Distortion of the liver mass can cause jaundice, encephalopathy and coagulopathy.
