Joan is a 52 y.o. Caucasian female, established patient. She is currently insured.
CC: 6 month DM f/u
HPI: She has no current complaints, except that she has been “more tired than usual over the past month.” Her last visit was 6 months ago for a DM f/u at which time here HbA1C was 6.8%.
PMH: Type 2 DM – 2013; HTN – 2014; Hyperlipidemia – 2014
FH: F: Deceased age 65 CAD w/first MI age 49. Type 2 DM, HTN, Hypercholesterolemia; Two brothers – Type 2 DM controlled on oral meds.
Social History: Produce manager at local supermarket. Married 2 children – ages 15 &18.
Social alcohol use, denies illicit drug use. Former smoker 20 pack years. Quit 5 years ago. Walks her dog 2 x daily ½ mile for exercise.
Medication: Metformin 1000 mg ER – 2 daily, Lisinopril 10 mg daily, Simvastatin 20mg daily.
Allergies: NKDA
Objective:
Biometrics: Ht: 64 in; Wt. 200lbs; BMI. 34.3.
VS: T: 98.4F; HR: 75; R: 18; BP: 150/92 mmHg; Waist circumference 40 in.
Labs results: CBC, CMP w/ no abnormalities. (WBC, H&H, LFTs and Renal functions WNL)
TSH 1.2
Lipids: TC: 200, LDL110, HDL 37, Trig 150
HbA1c 6.8
