A 19-year-old man is brought to his primary care physician’s office by his mother 
for concerns of his recent “bizarre behavior.” During the assessment phase, the RN 
learns that the patient has been observed to be talking to himself loudly in his 
room, refusing to attend his classes at the community college, and pacing while 
talking about Satan ruling the world and has been generally disorganized and 
inattentive to his personal hygiene. The RN performs a mental status examination 
as part of the routine assessment process.
