Assume that you are an individual support worker in CHISHOLM aged
care center. You have 03 patients to care for. The details of those 03
patients are as follows,

Scenario 01

 Mr. David, 78 years old client who is suffering from depression for
two months. His son and daughter in law visit him once a month.
When you are going as the morning shift support worker, he was in
upset mood and he refused the breakfast and morning drugs. As the
individual support worker how you can communicate with the patient
and plan the care according to organization’s policies and standards.

The next patient for you is Mrs. Jane, 75 years old lady with osteo-
arthritis and she has a medical history with diabetes mellitus and
asthma. She is having a severe pain in her two legs and she cannot do
her daily activities of living by herself. She suffers a lot with the pain.
Because of her severe pain she cannot engage with spiritual activities
as well. As the individual support worker how you can communicate
with the patient and plan the care according to organization’s policies
and standards.

The third patient for you is Mrs. Alen, 80 years old lady with a
breast cancer. She is in her end stages of life. She is so worried about
her life and she is hopeless. She has an unmarried daughter. Always
she is telling about her daughter’s life. As the individual support
worker how you can communicate with the patient and plan the care
according to organization’s policies and standards.
