Mr. R., a 93y/o male, presents as a direct admit to your medical-surgical unit in the hospital from the physician’s office. He presented at the physician’s office with shortness of breath, difficulty ambulating, leg pain and edema. Upon initial assessment, findings are:

Medications – ASA 81mg PO daily, Lasix 40mg PO daily, Lisinopril 10mg PO daily, KCL-20 1 tab PO daily, Albuterol Nebulizer 2.5 mg/5 ml Q4 hours PRN SOA
EKG – Sinus Tachycardia with the rate of 115
The patient states he has not been taking his “water pill” because it makes him go to the bathroom too much and it is hard for him to get around.
Health Hx – CHF, MI x2 with stents, Bilateral Total Knee Replacements, non-smoker
