45-year-old white male with a dry cough for the last 2 months. No other symptoms. Tells you he has been trying to exercise and lose weight since his annual physical. He has been monitoring his bp at home 3 days a week in the morning. BP runs mostly in 120s/70s.  

Past Medical Hx: HTN, Pre-Diabetic, Obesity, Seasonal Allergies, Anxiety and Depression 

Last annual physical with labs was 2 months ago showed new onset of HTN and Pre-diabetes. All labs wnl except A1C 5.9. Was placed on Lisinopril 5mg and told to start a healthy diet and lose weight.

Medications: Celexa 20mg, Lisinopril 5 mg, Zyrtec 5mg prn 

His vitals are BP-122/72, Temp-98.6, Resp-16, HR-70, O2-98%

Weight has decreased from the annual physical from 220 lbs. to 215 lbs. His height is 5″8. BMI: 32.7.

What is his diagnosis, and differential diagnosis? How will you prepare to assess this patient.  It is a problem visit so it will be a focused assessment.
