Why do you think some hospitals choose to ignore the ACT that was created?

 

In the video Attitude one of the speakers continuously mentioned the correct way to transfer him on and off the bed. He repeatedly spoke up for himself on the best transfer method for him and was repeatedly ignored and eventually harmed by this, why do you think his advocacy for himself was not enough to avoid harm to the patient?
