S3 noted. +2 pitting edema to bilateral extremities. Elevated BNP 5.1, Cholesterol 210 mg/dL, HDL 55 mg/dL, LDL 125 mg/dL, Triglycerides 150 mg/dL. CXR shows cardiomegaly and interstitial pulmonary edema. Other systems WNL
R – Diet restrictions (low salt 2 gms per day, fluid restriction 2 Liter per day), strict I &O, recommend flu and pneumonia vaccines, continue current medication regimen, monitor daily weights, follow-up with cardiologist, smoking cessation (medication, support groups), decrease cholesterol, respiratory therapy as needed.
