I: Mrs. Yu Yan McFarlane is a 38-year-old female.

S: She had an abdominal hysterectomy two (2) days ago. She has an indwelling catheter insitu, a wound drain, and an IVT. 

B: Yu Yan lives at home with her husband, her elderly mother, and 3 children, 3-year-old twins, and a 17-year-old son. Yu Yan suffers from anxiety and poor self-image. Yu Yan is embarrassed when undressing in front of nursing staff and is reluctant to ask for help. 

A: Yu Yan is on four (4) hourly observations. The most recent recorded observations were:

Temperature 36.5 °C
Respirations 22
HR 110
BP 135/80mmHg 
SpO2 98% on room air
7/10 abdominal pain

 Yu Yan has received opioid analgesia for two (2) days and has reported that she has not opened her bowels.

R: continue post-operative care, assess, monitor, and treat pain, gentle mobilization. One person assists with ADLs. Fluid diet.
