Prior to her hospitalisation, Mrs Smith lived in a single storey house with her husband and has one adult child who visits every other day. Vera’s husband provides assistance with care as required at home and she has no further assistance. She was self-caring when her health was stable. 

Following 10 days on the medical ward, Mrs Smith COPD has stabilised, however, she has deteriorated in comparison to her pre-admission condition. She is now on continuous oxygenation at 2L O2 via nasal prongs and cannot. She needs assistance with all levels of ADL’s.
