Reduced readmissions for patients with Congestive Heart Failure (CHF) can be addressed through a variety of DNP practice issues. There is a lot of opportunity for development in this area. To begin, I would investigate the overall CHF patient readmission rate at the hospital and the unit. This will provide me with an idea of the scope of the problem posed by readmissions and the degree to which there is space for improvement. It is possible to compute the readmission rate for CHF patients by calculating the number of readmissions that occurred within 30days of discharge and dividing that number by the total number of discharges.
Next, I would want to take a look at the percentage of patients who were readmitted to my unit due to CHF. This will assist me in determining which department may be experiencing the greatest amount of difficulty with readmissions and where my efforts may be required the most. To get the readmission rate for CHF patients treated on my unit, divide the number of patients who were readmitted within 30 days following their discharge by the total number of patients who were discharged from my unit.
Last but not least, I would want to investigate the particular factors that contribute to CHF patients being readmitted. By doing so, I will be able to identify prospective areas of action that could contribute to a reduction in readmissions.
