Directions: You will be creating a two-problem (two nursing diagnosis, one each page) care plan, step-by step. The first step is to read the case study below to “gather data”. Write down the data from the study, then group it according to potential “problem areas” that you can see (cluster the data). Choose one “cluster” of data that you think is a priority for this patient. You will build a care plan from this “problem area”…this data will help you chose a nursing diagnosis…. Case Study: Lucas Moreno is a 56-year-old Hispanic man who arrives at his employer’s health occupation clinic for his biannual health evaluation. Although he appears to be in general good health, his current weight is 25% above the ideal weight for his height and frame. His triceps skin fold measurement is 21 mm (normal is 12.5 mm), and his cholesterol level is 245 mg/dl (normal is <200 mg/dl). His 24-hour diet recall shows a diet high in fats and low in fruits and vegetables. He states that his lifestyle and job (as a computer programmer) are sedentary and that he has no regular exercise program. Lucas likes to watch movies and "surf the net." His father died at age 59 of a myocardial infarction. His mother is alive, but has diabetes and has had one large toe amputated. Lucas is a widower since his wife died in a car accident. He has no children and lives alone in an apartment above his mother's garage. His blood pressure is 158/98 and pulse is 75. When you bring up the subject of weight and lack of exercise, he is not opposed to discussing the possibility of losing weight and increasing his exercise. Lucas states, "I would like to learn more about good nutrition."
