Question 1. A patient has hypoxia. What measures will likely be part of the person’s care plan? Hypoxia occurs when cells do not have enough oxygen hence (hypo) oxygen (oxia). When hypoxia happens, cells cannot function properly as the brain is very sensitive to inadequate O2. This results in restlessness, dizziness, and disorientation as early signs. Hypoxia is high risk as it threatens a person’s life. To function at an optimal level, our body requires the necessary oxygen to be transported from our blood to our cells. To measure the oxygen concentration in arterial blood a pulse oximeter can be used to measure the oxygen concentration. Oxygen concentration is the amount of percentage of hemoglobin containing oxygen. A measure to use with a patient with hypoxia symptoms is to use the pulse oximeter to check the patient’s oxygen regularly to ensure that the patient can function optimally. Additionally repositioning the patient every two hours also allows for more blood flow and oxygen transported into cells. Repositioning and exercises should also be included in the plan to ensure adequate oxygen. Question 3.Define disability. Define rehabilitation. List the physical aspects of rehabilitation. List the psychological aspects of rehabilitation. A disability is referred to any lost, absent, or impaired physical or mental function. There are common causes that are seperated into two catagories. FIrst is acute, which is defined as a short course of time and the recovery will utimatley be completed and completely restoritive. Second, there’s chronic, which means it’s over a long course of time, the problem is controlled with treatment but can never be cured. Disabilities affect the little things like everyday activities that become mindless to able-bodied persons. Such as, eating, dressing, walking, and working ability. The degree of the persons disability affects how much functionality they have.Rehabilitation is the process of restoring the person to their highest possible level of physical, psychological, social, and economic function. Rehabilitation is a team effort, the person cannot do this recovery on their own. They need their family, friends, doctors, and health teams to help the person set goals and plan care with the focus on regaining function and independance again. Physical aspects of rehabilition could look like practing range of motion and stretching the muscles to build up the stamina again. Psychological aspects of rehabilitation can look like the person attending group therpy, one-on-one therapy, and having healthy coping mechanisms to help them learn how to handle their condition and their recovery. Question 10. Differentiate between glaucoma and cataracts. Provide information about the definition, signs and symptoms, persons at risk, causes, and treatment options. Describe nursing care measures on the care plan for a person with each vision problem Although both glaucoma and cataracts are eye problems, they are different. A cataract is better understood as clouding of the lens. Coming from the Greek word “waterfall,” it has been described as looking through a waterfall. This could occur on 1 or both eyes and surgery is the only treatment for this. Glaucoma on the other hand is fluid building up in the eye causing pressure on the optic nerve. This means that if the optic nerve is damaged, vision loss that could eventually lead to blindness occurs. Peripheral vision is lost and it’s described as like looking through a “tunnel.” Glaucoma has no cure and damage cannot be reversed. Taking care of a patient after cataract surgery includes, having them wear their eye shield, letting them know that they shouldn’t rub, press, or bump the eye, placing the over-bed table and stand on the un-operative side, and placing the call light within reach. Taking care of a person with glaucoma is pretty similar to taking care of a person with cataracts. Just understand the environment/setting the patient is in, communicate with the patient, understand your role as a care taker, and make sure the patient is well taken care of.
