Past Medical History

Untreated Hypertension x 2yrs

 

Family History

Father Died at 38 of Heart Attack.
Mother Died of Stroke at 50. Also had DM and HTN.

 

Abnormal exam findings listed :

Neurological: 

Alert and Oriented x4. 

Progressive symptoms.

Patient reports dizziness as “spinning sensation” 

Positive Romberg’s Test.  

Broad-based stance and gait.  

Stance and gait ataxia with falling towards the right side (ipsilateral). 

Sitting ataxia with falls to the right. 

Skew deviation – hypertrophic deviation on left.  

Double-vision x 1day,  on and off (diplopia)

Facial and hand numbness/tingling

New onset hiccups x 1day, constant

 

Musculoskeletal: 

Difficulty walking reported “leg heavy”

Strength is asymmetrical, weak on left hand grip and left leg raise compared to the right.
