Casey an RPN, works at Memorial General Hospital. She works on the surgical unit. Today one of the patients she is assigned to is Mrs. Scott, who was diagnosed with gastrointestinal cancer. Part of her treatment plan included extensive gastrointestinal surgery which she has 6 days ago. Mrs. Scott has not progressed well since her surgery. She had suffered significant amount of weight loss, a loss of appetite and severe nausea. Mrs. Scott’s physician, Dr. Miller is very concerned and has decided to insert a PICC line so that nutritional support – parenteral nutrition may be provided by this means. Dr. Miller contacts Casey in the nursing station and requests that she obtain Mrs. Scott’s consent for this procedure which is considered invasive. Casey completes the PICC line procedure form as Dr. Miller ordered and then goes to get consent from Mrs. Scott.
Casey informs Mrs. Scott about Dr. Miller’s concerns about her weight loss and lack of appetite and explains to her that he plans to insert a PICC line so that extra nutrients can be administered to her. Casey goes into more depth with Mrs. Scott explaining to her that these extra nutrients will help her with healing and also make her feel stronger and overall much better. In the past when Mrs. Scott has made decisions about treatments she has discussed these with her son as she feels that he knows the best thing to do. Mrs. Scott became very nervous and worried because her son was not visiting at the time. She said to Casey, “Do you know if this is going to hurt? I really am getting sick and tired of all the painful procedures and I am really not sure if I want to have any more treatments.” Casey responds with “Oh don’t worry about it, the doctor will make sure that you do not feel any pain.” Mrs. Scott tells Casey that she may want to wait until her son comes so she can discuss it with him. She tries to get a hold of him but is unable to reach him at work. Casey knows that Dr. Miller will be arriving soon so she says to Mrs. Scott, “You will be fine, do not worry. Dr. Miller is on his way here and wants this consent form signed, so can you please sign it now?”
