The following is my PICOT State/Question

P=Adult patients 21 and older diagnosed with hypertension

I=Medication Adherence, Disease education, and patient empowerment in managing treatment through change talk(MI), diet and exercise.

C=Non-adherence to medication, no changes in diet or exercise, the patient shows little interest in changing behaviors (sustain talk).

O=Reduction in readmittance to hospital for complications related to hypertensive crisis.

T= 6-month period

I need help identifying / wording the topic for my Capstone change proposal from my PICOT listed above and determining if my intervention is leadership or community intervention.

I am completing my practicum in a Community Health Primary  Care Clinic that monitors and tracks patients’ uncontrolled elevated blood pressures.
