I had to transcribe this from an audio clip (tried to send  the audio but not sure how) the underline words are what I can’t make out. If there is any way you could help it would be greatly appreciated.

This is a follow up note for your records on Mrs. Moynihan. As you recall, she had been hospitalized on 09/13 at Chambers hospital with severe right groin pain at a 4-5 hours duration.Â

She was a gravy to forafirafour who while during her pregnancies had have large painful veins in her right leg. She gave no history of previous superficial thrombophlebitis. Examination on admission to the hospital showed mars tendonitis over the right femoral vein, and a long saphenous vein from the groin to approximate were fine. There was no redness, edema, quimasign was negative, and there was obvious significant varicosity’s in no caspendas because of the severity or her symptoms, she was never less, treated as a superficial thrombophlebitis reviving the long saphenous vein extending to saphenofemoral junction. I felt at best if she be placed on bedrest, and treated with heparin. This was subsequently carried out during the next 10 days. Since that time, and following us with accumbent she has been monitored weekly, with prothrombin time and has been seen in the office. The plan would be to keep her on anticoagulants for 2 -3 months and then gradually taper off.
